About The Position

Assists with providing homemaker and/or personal care assistance in accordance with established policies and procedures and in support of related public health programs. Performs the direct service tasks assigned in the plan of care established by the RN and coordinates care with other disciplines.

Requirements

  • High School diploma, GED, or high school equivalent.
  • Certified Nurse Assistant (CNA) 75-hour course.
  • Valid driver’s license when driving any vehicle for work-related reasons.

Nice To Haves

  • Home Care Aide experience preferred.

Responsibilities

  • Provide physical support to assist clients to perform daily living activities such as bathing, dressing, toileting, standing, walking, or light exercise.
  • Remind clients to take medications.
  • Use safety measures to prevent incidents and instruct clients in home safety measures.
  • Notify RN on the same day of changes in client’s condition and document notification.
  • Demonstrate competency in all skills assigned by RN.
  • Give clients information on community resources and notify supervising RN of action.
  • Encourage clients to complete tasks as able for themselves.
  • Complete assigned household tasks in a courteous, efficient manner.
  • Comply with client’s needs in completing tasks according to their instructions.
  • Observe other tasks that could contribute to client’s safety and make suggestions to RN regarding care plan.
  • Provide clerical support for other public health programs by answering telephone calls, making appointments, entering data into computer systems, assisting with equipment requests and maintenance of loan closet.
