About The Position

UPMC Family Hospice is hiring a Home Care Aide for the Pittsburgh area. This role involves providing designated care under the direction of the primary service provider in the patient's home to achieve identified goals. The Home Care Aide functions as an extension of skilled nursing and/or rehabilitation services in the home.

Requirements

  • High School Diploma or GED
  • Minimum of 75 hours of Nurse Aide training (applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire if no previous training)
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire (Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR))

Nice To Haves

  • One (1) year experience
  • Certified Nurse Aide
  • Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located

Responsibilities

  • Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient’s condition to appropriate professional staff.
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Proven ability to facilitate change.
  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Participates in all required agency meetings.
  • Must maintain annual competency evaluation scores as determined by regulatory requirements
