About The Position

Park Nicollet is looking to hire a Home and Community Direct Supervisor to join our Home Care team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day. Position Summary: The Supervisor provides operational leadership for Home & Community Direct services, overseeing referral processing, scheduling, intake coordination, navigator roles, and other key initiatives that support timely and effective care delivery. This role ensures streamlined workflows, accurate processing, and strong collaboration with internal teams and external referral partners to meet patient and stakeholder needs. The Supervisor is focused on maintaining reliable systems, developing and implementing standards, and coordinating staffing and schedules to optimize service delivery. By driving process improvements and supporting clinicians in delivering high-quality care, this position plays a critical role in enhancing the patient experience and ensuring operational excellence across Home & Community Direct services.

Requirements

  • Bachelor’s degree Healthcare Administration, Management, Nursing, Physical Therapy or related field
  • 2+ years leadership/management experience in a healthcare/clinical setting
  • MN Registered Nurse
  • MN Physical Therapy
  • Excellent verbal and written communication skills.
  • Strong organizational skills.
  • Strong critical thinking, analytical and problem-solving skills.
  • Strong project management, team building and delegation skills
  • Strong knowledge of community resources, referral systems, and service delivery models.
  • Strong change management skills

Nice To Haves

  • Master’s degree in healthcare administration or related field
  • Experience in Home and Community Services
  • Experience with the EPIC EMR setting
