About The Position
Make an impact. Join our growing outpatient clinics, focused on improving the lives of vulnerable and underserved communities. We are seeking an eager and compassionate individual to support our growing community health center in improving screening for and preventing the spread of HIV/AIDs across our community of patients. The grant funded HIV Prevention Services Coordinator is responsible for facilitating St. Jude Neighborhood Health Centers successful participation in HRSA's Ending the HIV Epidemic Initiative. Through effective project management support system changes that advance quality and value of HIV prevention within primary care. Specifically, support the development and then the on-going strengthening of HIV prevention and care within our primary care setting. Plan, coordinate, and drive improvement in, HIV prevention & care along with overall grant project performance. Educate providers, caregivers and patients on issues related to HIV prevention and care. This position is responsible for program logistics and communications and entails direct engagement with patients. Ensures completion of project deliverables all within the scope, timeline, budget identified. Position promotes cost effectiveness and operational efficiency in the oversight of the projects and communications that align to the mission and values of St. Jude Neighbor Health Centers.
