Open Door Care Network provides health care and wellness programs to individuals and families in need throughout Westchester, Putnam and Ulster Counties. A pioneer Federally Qualified Health Center, Open Door provides more than 300,000 patient visits annually and serves nearly 60,000 individual patients who might not otherwise have access. For more information, please visit our website at www.opendoormedical.org. Open Door is seeking an HIV Prevention (PrEP) Navigator for its site in Port Chester, NY. This position is grant based thru 2022. As part of the medical care team, the services provided by the PrEP Navigator aim to improve patient health outcomes and assist patients in managing their own health care and becoming independent, and informed consumers of health care services. Primary responsibilities include the delivery of HIV prevention and navigation services offered to individuals at risk for HIV infection. These services seek to increase education on PrEP/PEP and improve linkage to care for individuals who would benefit from PrEP/PEP, HIV testing, and other forms of HIV prevention services. The PrEP Navigator improves access to services provided both internally and externally to Open Door by identifying and reducing barriers.

Requirements

Associate degree required, bachelor’s degree preferred.
Bilingual (English/Spanish).
Proficient in MS Office applications, including Excel.
Outstanding written and verbal communications skills with a culturally diverse patient and staff population.

Responsibilities

Identify potential candidates for PrEP/PEP, HIV testing, and/or prevention education through Open Door data reports, collaboration with medical providers and outreach activities.
Conduct outreach to individuals at high risk for HIV infection, especially young men who have sex with men (MSM) and transgender individuals and assist those who are eligible and interested in PrEP with navigating the process of obtaining and maintaining a PrEP regimen.
Provide education to clients on prevention topics, including PrEP/PEP, consistent and correct condom use, safer sex practices, risk reduction, regular STI and HIV testing, and the importance of mental health and substance use issues in relation to HIV risk.
Provide assistance with appointment scheduling, accompaniment, and appointment reminders as needed.
Provide referrals to insurance enrollers and assist with enrollment into medication assistance programs.
Participate in the development of LGBTQ-affirming and culturally competent outreach materials for the targeted communities.
Conduct group presentations at local community based organizations and health and human service agencies to increase knowledge of PrEP and engage potential PrEP clients.
Provide support around linkage to other services needed both internal and external to Open Door, such as mental health care, substance abuse counseling, employment services, support groups, and concrete needs.
Integrate PrEP assessment and navigation into the medical practice and assist medical providers to insure that PrEP services are provided efficiently and seamlessly as part of the workflow on the medical units.
Provide care management that facilitates, coordinates and documents participant-centered care for participants across Open Door teams and departments to better address multiple and co-occurring concerns.
Maintain electronic medical records in compliance with Open Door’s standards in a timely manner.
Meet PrEP screening, clinical assessment, follow-up, patients on PrEP, retention and other goals and targets as set forth by grantors and Open Door.
Maintain program data and evaluation metrics and deliver reports and completed data entry accurately and in a timely manner.
Ensure performance improvement initiatives are supported with timely and accurate data and consistent actions.
