Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis. The HIV Care Coordinator provides a specialized form of non-medical case management to people living with HIV (PLWH). This position works to link, engage and retain PLWH in medical care and HIV services by developing individualized care plans that includes medical, psychosocial, financial, and other supportive services. The primary goal of HIV Care Coordination is to ensure continuity of care, promote self-sufficiency, and to enhance the quality of life of PLWH by facilitating entry and long-term engagement into HIV care.
