The HOPWA Case Manager is responsible for providing comprehensive case management and supportive services to individuals and families living with HIVAIDS who are or at risk of homelessness This role involves assessing client needs developing individualized service plans and addressing barriers to housing stability The Case Manager ensures compliance with HOPWA eligibility requirements while connecting clients to stable housing healthcare and other community resources The ultimate goal is to support housing stability promote equitable access to services and provide ongoing advocacy and support to enhance clients quality of life
