About The Position

The HIMS Clerk at Unity Health Care is responsible for managing the receipt, tracking, processing, maintenance, and retrieval of patients' medical consult reports and referrals. This role serves as a liaison between hospitals and specialists, ensuring that consult reports are received, reviewed, and properly filed. The clerk will also handle patient communications regarding appointments and maintain compliance with HIPAA guidelines.

Requirements

  • High school diploma or equivalent.
  • One year of clinical office experience.
  • Ability to understand medical terminology.
  • Ability to work both independently and as a team member.
  • Good organization and communication skills.
  • Demonstrated ability to effectively work and communicate with diverse populations.
  • Demonstrated proficiency with business software (e.g., Microsoft Office Suite).
  • Knowledge of electronic medical records strongly preferred.

Responsibilities

  • Screen, track, obtain, and maintain patients' referrals and medical records.
  • Act as a liaison between hospitals and specialists to obtain reports.
  • Utilize team-based communication strategies to close the loop on referrals.
  • Call patients when appointments are missed to inquire about the reason.
  • Reschedule missed appointments in eCW.
  • Fax patient medical records to specialty facilities as needed.
  • Send notifications to providers of outstanding referrals.
  • Document patient encounters to avoid losing patients in the follow-up process.
  • Send certified letters to patients if unable to reach them.
  • Ensure consult reports are assigned to the correct provider in a timely manner.
  • Follow up to ensure reports are reviewed and properly filed.
  • Send alert notifications to providers of reports received with high-risk results.
  • Update patient information as required.
  • Maintain accurate internal and external fax inbox phone numbers.
  • Manage fax inbox by assigning documents to the proper provider.
  • Assist with population management outreach to patients by phone.
  • Review and assign medical record requests to Datafile Technologies for processing.
  • Keep Health Center Director, HIMS Supervisor, and providers informed of problems with patient records.
