The position involves retrieving charts from nursing units as needed, accounting for all discharges, and searching for missing charts. The role includes preparing and scanning medical records, performing quality control (QC) and validation on scanned records to ensure the highest quality of legibility and accuracy. Additionally, the position requires interviewing patients for birth certificate completion according to the Center for Vital Statistics, initiating fetal death certificates, and completing death certificates for all hospital deaths in accordance with guidelines. The individual will index and scan paper medical records into the electronic health record, after removing documents that are not part of the permanent record. Responsibilities also include preparing medical records for shipment to a secure storage facility, purging permanent records in accordance with state and hospital regulations, assigning documents for electronic signature, and analyzing charts for physician deficiencies to report to the HIM Department Supervisor. Other duties may be assigned by the supervisor.
