About The Position

This position drives high-value care practices, focusing on optimizing clinical alignment, resource effectiveness, and improved outcomes for identified patient populations. This role focuses on improving care reliability, reducing unwarranted variation, and aligning care delivery with value-based healthcare principles. Working across the continuum of care, this role collaborates with interdisciplinary teams, and leverages data-driven insights to improve clinical outcomes while minimizing costs. The role coordinates and leads teams across the organization to establish, measure, implement, and evaluate large-scale improvement projects to address complex opportunities across the CentraCare organization. Reinforces and amplifies culture of high reliability through expert knowledge of improvement science to support efficient, reliable, consistent, and repeatable processes and workflows resulting in optimal staff and patient safety, quality, operational, and financial outcomes. Team member is a curious and continuous learner, engaging in development opportunities to remain current in subject area.

Requirements

  • Bachelor's degree in finance/accounting, business, or healthcare related field or 5 years of leadership or improvement experience required.
  • 3 years minimum of continuous improvement or bundled payment initiatives and value-based care models in acute care settings experience required.
  • Strong interpersonal, organization, analytical, and multitasking skills required.
  • Strong change management and project management skills.
  • Ability to think creatively and work collaboratively to address problems.
  • Ability to periodically travel among sites as needed.

Nice To Haves

  • Certified Professional in Healthcare Quality (CPHQ) through the National Association for Healthcare Quality (NAHQ).
  • Expertise in health care data sources, including claims data, electronic health records (EHR), clinical data, and patient-reported data.
  • Experience in developing and implementing clinical protocols, care pathways, and evidence-based practice guidelines.
  • Experience in continuous improvement specific to healthcare, utilization management, or clinical profession.
  • Experience with project management, Model for Improvement, Lean, Six Sigma or other improvement methodologies.
