Established in 1980, the Greater Lawrence Family Health Center (GLFHC) is a multi-site mission-driven non-profit organization employing over 700 staff whose primary focus is providing the highest quality patient care to residents throughout the Merrimack Valley. Nationally recognized as a leader in community medicine (family practice, pediatrics, internal medicine, and geriatrics), GLFHC has clinical sites throughout the service area and is the sponsoring organization for the Lawrence Family Medicine Residency program. GLFHC is currently seeking a temporary High Risk Obstetric (HROB) Nurse Care Coordinator. The HROB Nurse Care Coordinator serves as an integral member of the healthcare team and is responsible for coordinating the care of high risk OB patients from initial entry into care through the first year of the infant’s life including postpartum visits, to ensure the optimum health of these high risk patients. The role promotes continuity of care for OB patients and newborns, and serves as a resource to clinicians and other clinical staff involved in prenatal care. The role will focus on facilitating timely entry to prenatal care, and identifying and coordinating appropriate delivery of prenatal and newborn care as provided by the healthcare team, at the direction of the prenatal clinician. The role supports the scheduling and delivery of appropriate health care and community services during the pregnancy and post-partum periods, and initial well child visits for the newborn. This position reports to the Care Coordinator Nurse Manager (CCNM), and works closely and collaboratively with Clinical Care Nurse Coordinators (CCNCs), medical leadership, and nurses conducting prenatal intakes at all GLFHC sites.
