About The Position

Children's Outpatient Services provides treatment for children and adolescents and their families and is intended to improve and maintain a child’s or an adolescent’s stability in the community. Treatment focuses on problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Services include individual, family and group therapy, medication evaluation and management, case management, in home intervention, case consultation and summer day treatment services. The type of treatment is highly variable and depends upon the individual’s existing symptoms and developmental level.

Requirements

  • Bachelor’s degree preferred in Social Work (BSW)
  • Must be 23 years of age or older with a valid Alabama Driver’s License and proof of current liability insurance
  • Knowledge in adult specialty to be attained within (6) months of employment, including knowledge of psychiatric disorders in the adult population, Intellectual and Developmental Disabilities, Substance Use Disorders, Housing programs, VA programs, area shelters, Intellectual Disability placement services, inpatient substance abuse services, nature of addiction for referral purposes, behavior management, crisis intervention, community resources, nursing home and assisted living facility admission requirements, cultural diversity, educational methods appropriate to adults with psychiatric disorders, psychiatric rehabilitation principles, involuntary commitment laws, procedures and court policies

Responsibilities

  • Completes a comprehensive home-based assessment of the consumer’s needs and strengths utilizing the CANS within 14 days of intake
  • Identifies goals and plans for appropriate services based on the CANS
  • Develops Individualized Service Plan (ISP) within 30 of first face-to-face meeting
  • Intensively collaborates with the treatment team members and outside sources in the delivery of care coordination
  • Provides appropriate linkage to community resources
  • Provides ongoing monitoring of consumers’ progress and needs as assessed in the CANS
  • Advocates on behalf of the consumers to secure community resources
  • Evaluates the consumer for discharge planning
  • Effectively demonstrates the application of child/adolescent specific competencies
  • Effectively manages caseload based on consumer needs
  • Meets AltaPointe productivity standards
  • Seeks clinical supervision and consultation as needed
  • Accepts and employs suggestions for improvement
  • Actively works to enhance knowledge and skills
  • Documents in a timely fashion per AltaPointe policy
  • Documents appropriate, complete, and comprehensive CANS assessments
  • Documents measurable and specific goals in the individualized service plan
  • Documents in a clear and concise manner, types of consumer problems, ISP goals addressed and services provided
  • Documents case disposition, case closings, follow-up and referrals
  • Documents legibly
  • Treats consumers with care, dignity and compassion
  • Respects consumer’s privacy and confidentiality
  • Is pleasant and cooperative with others
  • Assists consumers and visitors as needed
