About The Position

Title : High Fidelity Wraparound Facilitator Location : Northern KRCC Counties (Breathitt, Lee, Owsley, Wolfe) Employment Type : Regular Full-Time Credentials : Bachelor’s Degree in an approved Human Services field. One year of experience providing TCM services to children/youth with SED and their families. Special Skills/Knowledge Required : Willingness and ability to complete HFW Facilitator training. Must maintain a valid driver’s license with acceptable driving record throughout employment. Must have an extensive knowledge of community resources. Primary Functions : Responsible for completing a comprehensive strengths-based assessment of the individual and family. Ensures transportation needs are met as necessary to meet treatment needs. Complete service documentation and billing entries. Participate in skill enhancement activities, both as participant and provider. Obtain required continuing education hours and other training as necessary to meet state, agency and credentialing requirements.
