About The Position

The Home Health Aide/CNA carries out supportive duties for the Nursing Department of a health care provider by performing specified, non-clinical skills under the direction and supervision of a Registered Professional Nurse, or other Agency designated health care professional. If assigned to a psychiatric patient, the aide/CNA must have documented experience in additional training to meet the needs of this specialized population. OneHome coordinates a full range of post-acute care including home health, infusion therapy, and durable medical equipment services at patients’ homes. Requirements

  • CNA/ HHA active License
  • CPR certification
  • Current State License or Registration as a Certified Nursing Assistant (as required by state)
  • Documented experience in additional training to be able to meet the needs of psychiatric patients (if assigned to a psychiatric patient)
  • Be screened for TB (if selected for this role)
  • Valid state driver’s license
  • Proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
  • Excellent customer service
  • Strong clinical background
  • Strong sense of engagement to be a team player
  • Strong written and verbal communication skills
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Responsibilities

  • Provides assistance with personal care, hygiene, and activities of daily living.
  • Encourages client participation in activities to the extent to which the client is able.
  • Assists with ambulation as instructed.
  • Assists client to: a) bed, b) commode, c) chair.
  • Turns and positions bed bound clients.
  • Measures and records intake/output, as assigned.
  • Measures and records temperature, pulse and respiration on each visit.
  • Changes bed linen if needed.
  • Prepares simple meals following dietary instructions as instructed.
  • Maintains a neat and clean environment.
  • May grocery shop one time a week for list of ten items or less as needed.
  • Informs supervisor of any changes in client’s condition or home situation.
  • Follows care plan as written.
  • Provides documentation of care given on Agency approved forms.
  • Reports any changes in patient’s condition, living conditions etc., to RN/Supervisor as they occur.
  • Performs any other task/duty that is specifically assigned by supervisor, and for which aide has been specifically trained.
  • Conducts self in a professional manner at all times and in all situations.
  • Provides Agency with: a) required certificate, and b) necessary information to be able to verify experience, c) documentation of continuing education.
  • Accepts only those assignments for which he/she qualified.
  • Complies with all Agency policies and procedures.
  • Communicates with Agency about any problems or concerns.
