About The Position

Hospice of the East Bay (HEB) is a community-based nonprofit that provides the hospice and palliative care services that support and comfort people and their loved ones through the final stages of life. Reporting to the In-Patient Facility Manager, this position is responsible for carrying out the duties assigned in order to increase the patient’s level of comfort and to maintain personal hygiene and a safe healthy environment for the patient. Although the Home Health Aide reports to a primary manager, HEB expects the Home Health Aide may work in more than one program or department, and assignments are given to best meet the needs of our patients.

Responsibilities

  • Performing personal care needs after given a patient’s assignment and attending to his/her requests.
  • Providing patients and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.
  • Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails).
  • Following the plan of care as written by the RN.
  • Demonstrating clinical care as defined in the Home Health Aide scope of practice.
  • Assessing and reporting on the patient’s condition and significant changes to the RN.
  • Recognizing and reporting emergencies.
  • Keeping the patient’s living area clean and orderly, as assigned.
  • Adhering to the organization’s documentation and care procedures and standards of personal and professional conduct.
  • Participating in quality assessment performance improvement teams and activities.
  • Participating in IDT meetings.
  • Respectful of patient and family/caregiver environment and patient’s personal needs.
  • Maintaining inventory of disposable supplies.
  • Maintaining cleanliness and infection control standards.
  • Performing other duties as assigned.
