About The Position

The Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) is responsible for administering nursing care under the supervision of a registered nurse for clients in their place of residence. The role involves coordinating care with the interdisciplinary team, clients, their families, and the referring agency. The LVN/LPN will have contact with patients of all ages in a clinical setting and must demonstrate appropriate behavior when interacting with patients across all populations.

Requirements

  • 1-year experience as an LVN/LPN in a clinical care setting.
  • Completion of an accredited Licensed Vocational/Licensed Practical Nursing program.
  • Current license in good standing to practice as an LVN/LPN in applicable states.
  • Current CPR certificate.
  • Current driver’s license in good standing.
  • Nursing skills as defined by generally accepted standards of practice.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Proven decision-making skills.
  • Ability to read, write, and comprehend English.

Nice To Haves

  • Preferred home health experience.

Responsibilities

  • Promote and exemplify the company mission, vision, and values at all times.
  • Assist in identifying the client’s physical, psychosocial, and environmental needs under the direction of the RN.
  • Participate in planning and implementing care in conjunction with the RN, in accordance with the Plan of Care (POC).
  • Assist clients in the recovery or maintenance of life functions and prevent illness and relapse of illness.
  • Provide care utilizing infection control measures that protect both client and staff.
  • Ensure continuity of care through the delivery of quality patient care.
  • Communicate effectively with clients, their families, team members, and other health care professionals.
  • Monitor assigned cases to ensure compliance with requirements of third-party payers.
  • Complete appropriate documentation in a timely manner to assure compliance with company policy.
  • Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.
  • Meet mandatory continuing education requirements of the Agency/licensing board.
