Position Summary The Clinical Manager, reporting to the Administrator and/or Director of Clinical Services, is responsible for ensuring high-quality, compliant client care in alignment with the Agency’s mission, policies, and regulatory standards. This role provides clinical leadership through planning, coordination, implementation, and evaluation of home health services, while supervising and supporting field staff. The Clinical Manager collaborates closely with the interdisciplinary team, patients and families, and referring agencies to ensure effective, patient-centered care delivery.

Requirements

  • Current licensure and registration as a Registered Nurse (RN).
  • Minimum of two years of experience as a Community Health Nurse or Registered Nurse in a certified home health agency.
  • Demonstrated leadership experience, including fiscal and operational oversight.
  • Strong knowledge of home health regulations, licensure requirements, and Medicare Conditions of Participation (CoPs).
  • Excellent communication, interpersonal, and decision-making skills.
  • Strong nursing skills consistent with generally accepted standards of practice.
  • Ability to read, write, and comprehend English.
  • Hepatitis profile required if making patient visits.
  • Office-based work with prolonged sitting and use of standard office equipment.
  • Ability to lift, bend, stand, and move equipment as needed.
  • If making patient visits, ability to stand and walk for extended periods, lift up to 50 pounds, and assist with patient mobility.
  • Ability to work under pressure, manage deadlines, and handle stressful situations calmly and professionally.
  • Flexible schedule required, including availability for PRN emergency call.
  • Local travel required; reliable transportation and auto liability insurance required.
  • Possible exposure to inclement weather, bloodborne pathogens, bodily fluids, and infectious diseases during patient visits.
  • Participation in required continuing education and mandatory training programs.
  • Commitment to ongoing professional development to enhance job-related knowledge and skills.
  • Willingness to perform additional duties as assigned.

Nice To Haves

  • Bachelor’s degree in nursing or a health-related field preferred.
  • Previous healthcare management or supervisory experience preferred.

Responsibilities

  • Plan, coordinate, manage, and evaluate the activities of assigned clinical teams to ensure high-quality home health services that meet organizational standards and comply with federal, state, and local regulations.
  • Supervise, coach, and support assigned field staff to promote efficient, cost-effective, and quality-driven care delivery.
  • Oversee the implementation of patient care plans, including recertifications, discharge planning, and continuity of care.
  • Coordinate and lead regularly scheduled multidisciplinary case management conferences.
  • Ensure timely, accurate, and compliant clinical documentation by assigned staff.
  • Monitor staff productivity, performance, and time management, providing ongoing feedback and corrective action as needed.
  • Assess staffing patterns and workload using performance indicators to maintain appropriate staffing levels.
  • Supervise patient visit scheduling to ensure timeliness and quality of services.
  • Serve as a clinical and regulatory resource for staff regarding policies, procedures, regulations, and coverage issues.
  • Maintain open and effective communication with all levels of staff and management.
  • Direct clinical staff to appropriate internal and community resources to support patient care.
  • Conduct in-home supervisory visits as needed and in preparation for annual evaluations.
  • Complete annual performance evaluations in a timely, constructive manner.
  • Assist with staff orientation, education, and ongoing development programs in collaboration with the Quality and Clinical Excellence Department.
  • Collaborate with the management team on organization-wide initiatives, including participation in the Performance Improvement Plan.
  • Perform additional duties as assigned, including providing direct patient care when necessary.
