That is where we step in. This position provides basic social services to individuals and/or families participating in the Healthy Start Program in Hernando County. The responsibilities include case management for Healthy Start clients and education, and ensuring clients receive appropriate referrals and services in compliance with Healthy Start Guidelines. Funding for this position is provided by the Healthy Start contract to improve pregnancy outcomes.

Requirements

  • A bachelor’s degree in Social Sciences, Social Work, Human Services, or a Health-Related Field such as Health Education, Child Maternal Health, or Public Health is required.
  • Valid Florida driver’s license and insurable by the agency’s current insurance carrier.
  • Must successfully clear Level II Background Screen
  • Must possess an acceptable driving record and have reliable transportation
  • Excellent knowledge of community resources
  • Excellent interpersonal and communication skills (verbal and written)
  • Excellent computer skills in Microsoft Office Suite (Word, Excel, PowerPoint)
  • Effective public speaking skills
  • Ability to adapt to a hybrid working environment as needed
  • Ability to travel up to 75%
  • Ability to lift 10-25 pounds
  • Ability to safely operate a motor vehicle
  • Ability to work evenings and weekends as needed
  • Ability to work in an environment with frequent interruptions and a moderate noise level
  • Ability to independently complete job duties within required time frames
  • Knowledge of family and systems theory
  • Bilingual (Spanish)

Nice To Haves

  • 2 years of related experience is preferred.
  • Experience conducting home visits with a similar population is preferred.
  • Knowledge of obstetrics, pediatric health, and health care systems preferred

Responsibilities

  • Bilingual in Spanish and English
  • Perform case management services for individuals/families receiving Healthy Start services.
  • Assist clients in accessing community services and provide information and education to clients about smoking cessation, childbirth education, breastfeeding support, interconception education and reproductive life planning, parenting, and infant growth and development.
  • Initiate tracking procedures for the client, family of the client, and the client’s provider to manage appointments and referrals for services.
  • Assess the educational needs of Healthy Start clients who are at risk for poor pregnancy outcomes.
  • Assist with providing office coverage and be available to receive any phone calls or assist any visitors to the Healthy Start Office during required office hours.
  • Complete all Healthy Start assessments and home visits as required per program guidelines.
  • Maintain/update client files and ensure the Healthy Start database is updated as required per policy and Healthy Start Standards and Guidelines.
  • Assist in conducting outreach activities to providers and other community organizations to promote the Healthy Start program.
  • Participate in quality improvement and quality assurance activities of the program to enhance services.
  • Prepare and facilitate group connections as required. Encourage clients to attend.
  • Conduct client satisfaction surveys as related to the Healthy Start guidelines and protocols.
  • Complete documentation and accurately code for Healthy Start services in WFS within 72 business hours.
  • Complete initial assessments as required by the Healthy Start Standards and Guidelines and core performance measures.
  • Attend meetings, classes/conferences, and in-service programs as requested or assigned to update knowledge and skills related to the Healthy Start Program.
  • Must maintain positive working relationships with other community agencies and medical providers.
  • Assists with the completion of monthly and quarterly reports.
  • Complete all job duties and case management tasks within the required times.
  • Other duties as assigned.
