About The Position

The Hepatitis C Navigator at Friends For All is a full-time role focused on providing HCV prevention services and linkage to care for vulnerable populations, particularly those with substance use disorders in Shelby County. The position involves community outreach, testing, education, and support for individuals living with Hepatitis C, aiming to reduce stigma and improve health outcomes.

Requirements

• High school diploma or GED (bachelor's preferred).
• Peer recovery specialist certification strongly preferred.
• Experience working with people who inject drugs preferred.
• Two years' previous experience working with HIV/AIDS population preferred.
• Experience with evaluation web, redcap, and NBS preferred.
• Excellent verbal and written communication skills.
• Ability to operate with purpose, urgency, and accuracy in a fast-paced environment.
• Strong relationship-building skills with diverse populations.
• Computer skills including MS Office 365, especially Outlook and SharePoint.

Responsibilities

• Provide rapid point of care HIV, HCV, and Syphilis testing in an outreach environment.
• Offer risk reduction education and referrals to clients.
• Identify social determinants of health issues and provide appropriate referrals.
• Provide Hep C navigation services at local syringe service programs and substance use programs.
• Schedule clients for confirmatory RNA testing and link them to treatment.
• Assist clients with applying for patient assistance and co-pay assistance programs.
• Educate clients on harm reduction services and connect them to community resources.
• Occasionally drive to and set up mobile clinics at outreach events.
• Complete accurate and timely documentation of services in required databases.
