About The Position

Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services and programs addressing the social determinants of health such as housing and employment. Aspire has health centers in five Central Indiana counties serving Hoosiers of all ages and walks of life. The Hepatitis C Care Coordinator will support people living with Hepatitis C by providing them with care coordination and linkage to medical treatment. This position will assess eligible patients for needs, identify barriers to hepatitis treatment, and then connect patients with resources to reduce or eliminate those barriers. The Hepatitis C Care Coordinator will work closely with other departments, community resources, and clinics to help their patients reduce barriers and receive treatment. This position serves a multi-county region and the hours for this position are dependent on funding.

Requirements

  • High School Diploma or Equivalent required
  • Minimum one (1) year experience in public health, social work, insurance enrollment, counseling, or related field preferred
  • Experience working with diverse populations preferred
  • Must have knowledge of and ability to relate to community resources in order to best serve clients
  • Must have reliable transportation to transport self/others and to attend trainings and/or meetings
  • Must possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization
  • Must be able to occasionally work evenings and weekends
  • Strong ability to maintain strict confidentiality and handle highly confidential information with professionalism

Nice To Haves

  • Bachelor’s Degree in a Human Services related field from a college/university accredited by the US Department of Education preferred
  • Basic knowledge of HIV, Hepatitis C, and Harm Reduction Principles highly preferred
  • Knowledge and understanding of Indiana Medicaid, HIP 2.0, and Marketplace coverage highly preferred
