About The Position

This posting is for a Hematology RN Case Manager in the Children's MN Cancer & Blood Disorders Clinic. A hematology case manager is responsible for coordinating and overseeing the daily care of children with blood disorders to ensure their treatment plans run smoothly and safely. On a typical day, they review patient charts and treatment orders, coordinate appointments for transfusions, labs, and specialty visits, and monitor patients for complications or side effects. They serve as a central point of communication between providers, patients, and families, provide education on treatments and home care, arrange referrals and support services, and advocate for patient needs. Throughout the day, they document care, collaborate with the multidisciplinary team, and work to ensure no aspect of the patient’s medical or psychosocial care is overlooked.

Requirements

4 year degree BSN/BAN
Current MN RN license.
Current BLS Certification.
APHON Chemotherapy/biotherapy Certification obtained within 8 weeks of hire.
3 years of current Pediatric Hematology Oncology experience preferred.

Nice To Haves

CPON preferred.
3 years of current Pediatric Hematology Oncology experience preferred.
