About The Position

This position serves as a front-line pharmacist in the Specialty Care Center (SCC) in the TCRHCC main campus. SCC provides medical oncology, hematology, and infusion therapy services. The incumbent is responsible for providing a comprehensive pharmacy and medication management service to patients and ensures a consistent level of service in accordance with TCRHCC policies and in accordance with accreditation requirements. Participates in continuous improvement initiatives. Assesses systems/processes for efficiency and appropriateness of use. Provides patient care and services to neo-natal, pediatric, adolescent, adult, geriatric and obstetric patients. This position requires working rotational shifts: day, evening, nights, weekends, and holidays. The position also provides the opportunity for career advancement in pharmacy experience and certification.

Requirements

  • Doctor of Pharmacy or a Bachelor of Pharmacy degree from a school recognized by the American Council of Pharmaceutical Education
  • Must have a current valid and unrestricted license to practice Pharmacy in a State or Territory of the United States, or the District of Columbia
  • Must have and maintain a current valid certification of the following throughout employment: Basic Life Support (BLS) from the American Heart Association Advanced Cardiovascular Life Support (ACLS) from the American Heart Association, if none, must obtain within ninety (90) days of hire
  • Hematology/Oncology Pharmacist I: Two (2) years' experience as a Registered Pharmacists and one (1) year experience as a pharmacist in the inpatient and/or outpatient setting
  • Hematology/Oncology Pharmacist II: One (1) year experience in managing medications used to treat complex, chroninc, or rare medical conditions
  • A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in the following areas:
  • Possession of high ethical standards and no history of complaints and no prior records of substance abuse
  • Reliable and dependable; reports to work as scheduled without excessive absences
  • Maintains a positive professional attitude and demonstrates good interpersonal communication skills
  • Knowledge of legal dispensing practices, federal pharmacy regulations and therapeutic incompatibilities
  • Knowledge of sterile compounding (and USP 800 regulations) and non-sterile compounding procedures
  • Knowledge of drug-drug, drug-disease state, and drug-allergy incompatibilities
  • Knowledge of pharmacology, therapeutics of common disease states and treatment modalities, pathophysiology, and pharmacokinetics
  • Successful completion of and positive results from all background and reference checks, including positive employment references from authorized representatives of past and current employers demonstrating to the satisfaction of TCRHCC a record of satisfactory performance and that the applicant can perform the essential functions of the job.
  • Successful completion of fingerprint clearance requirements, physical examinations, and other screenings indicating that the applicant is qualified to be employed by TCRHCC and demonstrating to the satisfaction of TCRHCC that the applicant can perform the essential functions of the job.

Responsibilities

  • Reviews medical records for safety, appropriateness, and effectiveness of drug therapy with regard patient-specific factors (i.e. age, cancer stage, organ function, disease state), and consults with Medical Staff when concerns arise and collaborates with nursing staff to evaluate medication use.
  • Reviews, assesses, and inputs hematology, chemotherapy, infusion therapy treatment plans for SCC patients, verifies medication orders to ensure accuracy of dosage, treatment schedule, route of administration and safe administration based on laboratory information and evidence-based practices.
  • Counsel's patients, their representatives, and/or patient family members on safe and effective use of, including compliance with, prescribed medications.
  • Identifies and reports any medication errors or adverse drug reactions as per the Medication Error and Adverse Drug Reaction reporting policies, identifies areas in TCRHCC pharmacy or hospital practice that could be improved to minimize patient safety risks and provide more efficient operations.
  • Demonstrates knowledge of and competency in hazardous medication management, including the safe handling and preparation of compounds such as chemotherapeutic agents, biologicals, or other medications; packages, manufactures, and compounds products, including unit doses and intravenous additives (including chemotherapy), that are not commercially available as a premix.
  • Utilizes the relative health history and performs limited physical assessment related to the specialized pharmacy-managed clinics; distinguishes between normal and abnormal findings to recognize early stages of serious physical, emotional and mental problems.
  • Medications may be monitored, titrated and/or altered per specialized pharmacy clinic guidelines; therefore, prescriptions may be initiated, canceled, renewed and/or written to adjust doses. May order and interpret laboratory tests as appropriate for specialized pharmacy clinics and monitoring inpatient pharmacotherapy.
  • Participates, assesses, plans and implements interventions in pharmacy-managed clinics, in accordance with departmental and facility-wide policies and with the standards of professional pharmacy practice.
  • Serves as the liaison with clinical informatics team for electronic health record upkeep and collaborates with the Informatics Pharmacist to maintain drug formulary and infusion pumps.
  • Develops staff's pharmacological knowledge by participating in clinical programs, training pharmacy staff, students, interns, externs, residents, and health care professionals, and provides pharmacological information by answering questions and requests of health care professionals, including the significance of properties of drugs regarding absorption, metabolism and excretion in all age groups.
  • Maintains pharmacological knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies for continuing education (C.E.) required for specialized pharmacy-based clinics.
  • Ensures sterile compounding suite is compliant with USP standards.
  • Ensure proper PPE is always worn while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield.
  • Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (see CDC website for most current updates).
  • Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee, and external customer satisfaction.
  • All other duties as assigned.
