About The Position

The Help Me Grow Bexar County Care Coordinator plays a vital role in promoting the optimal development of young children by linking families to essential resources and services. This position focuses on system navigation regarding child developmental health, behavior, and learning, ensuring families receive the necessary support and care coordination. The Care Coordinator will also engage in community outreach to enhance awareness and understanding of the Help Me Grow program.

Requirements

  • Bachelor's degree in early childhood, child development, or related social service field required.
  • 2 years of work experience in early childhood development preferred.
  • Strong organizational skills, detail-oriented with the ability to prioritize and manage multiple projects.
  • Ability to build and maintain positive relationships with community agencies.
  • Professionalism, maturity, and demonstrated capacity for leadership are essential.
  • Demonstrate sensitivity and empathy regarding human service issues.
  • Prior understanding of the issues and struggles of families in poverty and experience working with low-income families.
  • Strong written, verbal, and interpersonal communication skills.
  • Ability to work with a wide variety of populations and personalities with respect, compassion, and dignity.
  • Proficiency in both English and Spanish oral and assessment skills is preferred.
  • Knowledge in both child development and economic mobility is preferred.
  • Must have a valid driver's license with reliable transportation.
  • Must pass Criminal Background Check.
  • Strong evaluation, problem-solving abilities, and decision-making skills.

  • Knowledge of Bexar County community resources preferred.

Responsibilities

  • Conduct system navigation through intake, screening, assessment, referral, and connection to early childhood services and community resources.
  • Support families by providing information and education on child development and Social Determinants of Health topics, making appropriate referrals to services both internally and externally, and providing continued support, advocacy, and follow-up as needed.
  • Administer and score Ages and Stages Questionnaires (ASQ), explain results to caregivers, provide families with age-appropriate developmental activities, address caregiver concerns empathetically using a trauma-informed approach, and provide appropriate referrals and follow-up.
  • Maintain a list of local programs, resources, and services that have the capacity to serve families.
  • Maintain communication with community providers regarding service availability and accessibility.
  • Document calls as required.
  • Network, actively engage, and collaborate with various community organizations and partners to improve early childhood services and resources.
  • Contribute to the development, implementation, and monitoring of an outreach plan targeting at-risk families with children ages 0 to 5 in Bexar County.
  • Collaborate with marketing and media teams to develop promotional materials and events.
  • Conduct outreach initiatives and participate in community events to promote Help Me Grow and generate awareness of UWSA strategies/programs.
  • Perform screenings or trainings on ASQ developmental tools and related topics.
  • Use technology to interact with clients and provide advocacy and follow-up services as necessary.
  • Compile and analyze data to identify community trends, strengths, and opportunities within the early childhood system.
  • Assist with the development of quarterly and annual reports.
  • Respect and maintain confidentiality; follow all agency and program guidelines regarding confidentiality.
