About The Position

The role of an Organ Transplant Coordinator is one that encompasses specialized nursing care focused on protection, promotion, and optimization of the health and abilities of the patient awaiting transplantation as well as the transplant recipient. This position is responsible for working with the multi-disciplinary team to coordinate, oversee, and monitor medical and psychosocial activities related to organ transplantation; including the collection, organization, and submission of statistical data.

Requirements

  • At least 1 year Cardiac RN related experience Required
  • Four-year Bachelor's degree in Nursing BSN Required
  • Current license to practice professional nursing in the State of Texas Upon Hire Required
  • Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements Upon Hire Required
  • PEARS/PACES or PALS (dept specific requirement)
  • Certified Clinical Transplant Coordinator (CCTC) within 24 months of employment and must complete continuing education requirements to maintain CCTC Required

Responsibilities

  • Evaluate transplant recipients and potential recipients by obtaining personal and medical history, reviewing diagnostic and laboratory test results, and scheduling medical/psychosocial consultations as warranted. Assist physicians with medical treatment, and provide plan of care information to patients and/or family members. Schedule clinic visits, labs, diagnostic studies, admissions, and emergency visits, and coordinate patient transport. Provide telephone triage of patient related concerns, reporting medical providers with patient status/condition changes as warranted.
  • Navigate, edit, and maintain the electronic medical record and other government/regulatory electronic data submission elements. Compile, calculate, and analyze statistical, medical, and operational data to report to government, regulatory, and medical organizations. Ensure the accuracy and completeness of government and regulatory required care plans and organ sharing forms.
  • Provide on-call, pre and post-transplant related coverage to address the needs of patients, hospital staff and outside organizations that is facilitated by the use of a rotating call schedule. Act as the liaison with organ procurement organizations. Maintain transplant waiting list and advise physicians, patients and/or families regarding eligibility status on the waiting list.
  • Develop and provide education regarding the transplant evaluation process and care while awaiting transplantation. Also provides ongoing education that begins during the initial transplant admission and continues throughout the patient's transplant related care at Children's.
  • Participate in intra and interdisciplinary programs and committees, attend organ selection committee meetings, quality assurance performance improvement (QAPI) review meetings, monthly departmental staff education meetings, and participates in orienting fellows/residents.
  • Responsible and accountable for prescribing, delegating and coordinating patient care. Uses clinical judgment based on nursing skills acquired through formal and informal experiential knowledge and evidence based guidelines to globally assess the patient's situation and through critical thinking and clinical decision making, develop an appropriate plan of care for the patient with the aim of promoting comfort.
  • Designated individual(s) may be cross trained to provide direct patient care to individuals admitted to the Dallas Transplant Treatment Room.
  • - This designated individual(s) will deliver competent and skilled care to patients according to their identified needs.
  • - This designated individual(s) will make continuous inquiries about the condition of the patient through the ongoing process of questioning and evaluating the situation and implements treatment changes if necessary, through collaboration with the health care team inclusive of patient and family.
