The Heart Failure nurse collaborates with the Heart Failure team in all aspects of patient care, with a focus on coordinating care needs of the heart failure population from the time of referral through end-of-life. Assists in the clinical management and resource allocation for patients with HF. Supports the growth and expansion of the Heart Failure program by actively participating in performance improvement initiatives to enhance care, processes, and outcomes. Coordinates and collaborates with other Heart Failure nurses in the market. Manages CardioMEMS and other remote monitoring. Uses critical thinking to triage patient phone calls and collaborate with medical providers to titrate medications within scope of practice. Provides evidence-based disease self-management education and serves as a resource for our heart failure patients. Accurately and timely documents in the medical record.
