The Clinical Pharmacist for the Heart Failure Program in Ambulatory Care is responsible for providing comprehensive medication management services for patients cared for by the Heart Institute, specifically, the Advanced Heart Failure Program. This role involves working collaboratively with physicians, advanced practice providers, staff, and other health care professionals to provide medication management, education, and care coordination across the health care continuum. The focus is on managing high-risk patients with chronic disease states, including heart disease, pulmonary hypertension, and advanced heart failure, through collaborative practice agreements addressing barriers to adherence and medication access, requiring additional educational needs, and benefiting from clinical pharmacist follow-up. The pharmacist is an integral member of the health care team, ensuring excellent patient experience, patient safety, best practices, and high-quality standards of care are maintained across the continuum of care.

Requirements

  • Currently licensed in good standing with the Board of Pharmacy in Idaho required.
  • Licensed in Oregon required within 6 months of hire.
  • Bachelor's Degree in Pharmacy and 3 years of experience may be considered in lieu of PharmD.
  • Minimum of 1 year of practice experience at patient care level required or equivalent.
  • Residency training: Post Graduate Year 1 preferred.
  • Board Certification in Pharmacotherapy or Ambulatory Care preferred.

Responsibilities

  • Implement evidence-based interventions and protocols for cardiovascular conditions.
  • Demonstrate leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, critical thinking, analysis skills and decision-making, priority setting, work delegation, and work organization.
  • Monitor therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions.
  • Work with health care team to optimize pharmacotherapy opportunities, formulary adherence, reduction in polypharmacy and associated drug cost.
  • Demonstrate knowledge of clinical pathways for heart failure patients.
  • Review and act upon automated surveillance and clinical decision support tools.
  • Support department and system-wide requirements for compliance with Saint Alphonsus Health System policies, regulatory and accreditation-related programs, and quality/performance improvement efforts within the organization.
  • Provide guidance and direction for patient education and medication safety.
  • Identify high risk patients as defined by the evidence-based literature or from internal databases including patients with complex medication history, multiple chronic conditions, and or identified as having suffered complications of an adverse drug event.
  • Review identified patients' medications and clinical conditions for complication-prone regimens, redundant medications, overt contraindicated medications and allergies, or complicating substances such as herbal products and supplements and make recommendations.
  • Educate identified patients on the management of their disease states and appropriate medication use with emphasis on adherence and identifying/resolving medication related problems.
  • Assist with the transitions of care and related communication between acute and ambulatory care providers and between specialty care providers when multiple providers may be involved in a patient's care.
  • Support the development and implementation of policies and procedures across the system consistent with practice and regulatory requirements.
  • Promote collaborative professional relationships with the medical staff and other healthcare professionals.
  • Participate in community/professional organizations as appropriate.
  • Model professional behavior and support all members of the care team in creating the environment for professional practice.
