About The Position

Collaborates with Physician and Interdisciplinary Team to implement plan of care and treatment for Advanced Heart Failure patients. Reviews medical record to anticipate clinical stability and to have a thorough understanding of the patient prior to follow up with the patient and physician. Ensures patient is progressing through clinical milestones and adjusting treatment plan as needed. Collaborates with Interdisciplinary team members to continuously identify more effective strategies to resolve barriers, reduce the severity of chronic disease, and avoid readmission. Escalates barriers to the plan of care (clinical, social, and environmental) through appropriate channels to resolution. Works in partnership with physicians to consider alternate levels of care if patient is not meeting clinical stability. Collaborates with Social Work to manage the needs of patients who have high acuity psychosocial needs coupled with need for medical care to mitigate risk for readmission. Responsible for performing job duties in accordance with the mission, vision and values of Tampa General Hospital. In addition to the above duties, the Heart Failure Care Coordinator provides effective clinical health coaching to assist patients with self-management of their chronic disease and lifestyle changes to mitigate health risk.

Requirements

  • Graduate of accredited School of Nursing; licensure to practice as a Registered Nurse by the State of Florida
  • Three years nursing experience in acute care or ambulatory setting.
  • Specialty Area of practice qualifications include graduate of accredited School of Nursing, licensure to practice as a Registered Nurse by the State of Florida.

Responsibilities

  • Demonstrates current knowledge and expertise required to successfully perform disease management and care coordination for a chronically ill patient population.
  • Reviews medical history, physical conditions, medical diagnosis, available treatment options, financial resources and physician orders comparing to plan of care in order to make necessary revisions.
  • Refer patient to appropriate ambulatory, hospital, and community resources
  • Participates in department and organizational performance improvement activities; performs duties according to established procedures, protocols and professional standards.
  • Provides accurate and consistent documentation of services including assessment, care plans, education and reassessment, and involvement with physicians.
  • Participates in the orientation and education of new employees and students.
  • Demonstrates appropriate verbal and written communication skills in interactions with all customers.
  • Utilizes Culture Care and Showtime Principles in interactions with all customers.
