About The Position
Implement and administer policies, programs and activities. Responsible for performing psychosocial assessments on family's ability to meet their child's healthcare needs. Ensure the preparation and maintenance of required documentation per accreditation standards and CMC's policy.
Requirements
- At least 2 years previous Social Work experience
- Graduate or professional work or advanced degree; or equivalent experience
- State of Texas LMSW license
Nice To Haves
- At least 4 years related experience
- State of Texas LCSW license
Responsibilities
- Completing in-depth psychosocial transplant evaluations on patient families to access risk and benefits of moving forward for transplantation
- Assisting heart transplant patients and families adjust to acute and ongoing medical stressors
- Facilitate parent/child adjustment to illness and hospitalization.
- Provide ongoing evaluation and treatment services to heart transplant patients and families with complex psychosocial needs
- Coordinate anticipatory discharge to ensure that patient's home - care is prearranged prior to leaving the hospital.
- Coordinate and direct multidisciplinary patient care conferences focusing both on medical and biopsychosocial needs.
