About The Position

The Hartford HealthCare Center for Healthy Aging serves as a resource and assessment center, providing essential information and services to seniors, their loved ones, and caregivers to enhance their quality of life. The Healthy Aging Navigator is responsible for overseeing and managing care transition operations for older adults within a specific region of the Hartford HealthCare system. This role acts as a lead representative of the Hartford HealthCare Community Network, ensuring timely access to appropriate services, reducing avoidable readmissions, and improving consistency in care transitions across various clinical and community-based settings. The Navigator also supports operational planning, performance monitoring, and program expansion in line with organizational priorities. Hartford HealthCare encompasses 8 acute care hospitals, a comprehensive Behavioral Health Network, Home Care services, Senior Services, and an extensive Medical Group network, serving over 23,000 lives daily across 185 towns in Connecticut. Requirements
- Bachelor’s Degree in Health & Human Service Field (Social work, Gerontology)
- Five years working in Healthcare and Care Coordination
- Exceptional Communication and Organizational skills
- Five years working with older adults
- Able to travel to locations throughout the region on a regular basis
- Strong analytical skills, able to work with and interpret large amounts of data
