About The Position

The Healthcare Navigator for Supportive Services for Veteran Families (SSVF) plays a crucial role in connecting Veterans to healthcare benefits and services, ensuring they receive timely and appropriate care. This position involves collaboration with a multidisciplinary team to address challenges in care coordination and provide equitable, client-centered support to Veterans with complex needs.

Requirements

  • High school diploma or GED certificate and 5 years of supervised clinical experience.
  • Bachelor's degree in Psychology, Social Work, or related field.
  • Bachelor's degree in an unrelated field and 2 years of supervised clinical experience.
  • Master's degree in Psychology, Social Work, or related field.
  • Experience with homeless and low income individuals with mental health or substance use issues.
  • Strong computer skills required, including comprehensive knowledge of Microsoft Office products.
  • Strong written and verbal communication skills.
  • Creative thinker with an adaptive personality.
  • At least 21 years old.
  • A valid driver's license, current car insurance, and daily access to your own car.
  • Willingness to provide transportation to clients.

Nice To Haves

  • Previous work experience with the Veterans Administration preferred.
  • Knowledge of or ability to learn other pertinent software products preferred.

Responsibilities

  • Act as a liaison between the SSVF grantee, the VA, community medical clinic, and other healthcare providers.
  • Coordinate the access of healthcare for Veterans with complex needs and who require assistance accessing health care services or adhering to health care plans.
  • Work closely with the Veteran's assigned multidisciplinary team, including medical, mental health, behavioral, and case management personnel.
  • Work within the SSVF team to provide timely, appropriate, client-centered care in an equitable manner.
  • Collaborate with the healthcare team and Veteran to identify and address system challenges for enhanced care coordination.
