Whether in the home or our inpatient hospice facility, our team provides not only expert care, but ongoing support, including bereavement services for loved ones. We’re seeking a Healthcare Liaison to help share that mission with the community. In this role, you’ll partner with physicians, nurses, and care teams to build awareness of hospice and palliative services and help ensure patients receive the right care at the right time. Reporting to the Director of Community Engagement, the Healthcare Liaison is responsible for promoting awareness and understanding of hospice and palliative care services within the medical and broader community. This includes providing education and information to physicians, nurses, social workers, and other healthcare professionals in hospitals, clinics, nursing and residential care facilities, and community organizations.
