The successful Healthcare Coordination Specialist will work with Resident case plans and goals and provides on-going assessment and support. Assists the residents to utilize community resources to meet individual needs. Coach residents to identify their skills and abilities to promote personal growth. Link residents with health care and care coordination services. We encourage candidates with lived experience to apply. This is an on-site position.
Requirements
- High school diploma or equivalent.
- Two-years work experience in a position that involved coordination among frequent interaction with patience/clients, health care providers, care coordinators, health plans, and other service systems critical to health.
Nice To Haves
- Bachelor’s Degree in Healthcare Management, Public Health, or related field.
