About The Position

Cooperative for Human Services Inc. is seeking a compassionate, organized, and clinically knowledgeable Licensed Practical Nurse (LPN) to join our team as a Healthcare Coordinator. In this role, the LPN will help coordinate and monitor healthcare services for individuals receiving support, provide staff training and consultation, support medication administration systems, and advocate for quality healthcare outcomes across our programs. Requirements

  • Current Licensed Practical Nurse (LPN) license in Massachusetts, in good standing
  • Experience in healthcare coordination, case management, community health, human services, or a related setting strongly preferred
  • Experience supporting individuals with intellectual and developmental disabilities is highly valued
  • Knowledge of medication administration practices, health monitoring, and general nursing principles
  • Strong communication, organization, and documentation skills
  • Ability to work collaboratively with a wide range of staff, providers, and families
  • Ability to travel between program locations as needed
  • MAP Trainer/Coordinator experience is preferred, or willingness to be trained if applicable

Responsibilities

  • Coordinate and monitor healthcare services and supports for individuals served, including review of medical records, identification of healthcare needs, and follow-up on recommended services, testing, treatments, and equipment
  • Maintain a current LPN license in the Commonwealth of Massachusetts
  • Support medication administration systems and serve as a resource regarding medication administration policies, procedures, and best practices
  • Provide consultation, support, and direction to program staff on healthcare-related concerns
  • Respond to health, medical, and safety-related issues, including providing on-call consultation and support as needed
  • Develop and deliver training to employees and, when appropriate, individuals served in areas such as: medication administration vital signs signs and symptoms of illness infection control nutrition and wellness seizure response safety practices
  • Conduct regular visits to program locations to audit healthcare supports and medication administration procedures
  • Attend medical appointments and consultations as needed
  • Collaborate with internal teams, families, guardians, service providers, healthcare professionals, and other stakeholders in a professional and supportive manner
  • Participate in ISP meetings as needed and advocate on behalf of individuals served
  • Monitor for potential health, safety, abuse, or neglect concerns and communicate appropriately
  • Assist in the development and implementation of healthcare-related policies, procedures, and quality initiatives
  • Prepare accurate and timely reports as required
  • Maintain confidentiality and uphold the dignity and rights of each person served
