About The Position

Care Coordinator & Community Relations – Home Care Location: Lakeland, FL or Winter Park, FL (Local Candidates Only) A growing home care agency is seeking a Healthcare Care Coordinator with strong relationship-building and business development experience to support client growth and daily operations in the Lakeland or Winter Park market. This role combines client intake and care coordination with community outreach and referral development. The ideal candidate is comfortable building relationships with local healthcare professionals while also managing client onboarding and caregiver coordination.

Requirements

  • Minimum 2+ years of experience in healthcare operations, care coordination, or healthcare sales
  • Demonstrated experience building referral relationships or supporting case growth
  • Strong understanding of healthcare service delivery
  • Must reside in Lakeland, FL or Winter Park, FL
  • Strong communication, organization, and relationship-building skills

Nice To Haves

  • Home health or home care experience
  • Experience in community outreach or healthcare business development
  • Familiarity with senior care referral networks

Responsibilities

  • Develop and maintain relationships with referral sources including assisted living communities, case managers, physician offices, and senior service providers
  • Support growth of new client cases through local outreach and relationship development
  • Manage new client intake, assessments, and service setup
  • Coordinate caregiver scheduling and ensure appropriate case coverage
  • Serve as a point of contact for clients and families
  • Maintain accurate documentation and operational records
  • Support retention of clients through strong communication and service follow-up
