About The Position

Pathfinder is seeking a compassionate and detail-oriented Healthcare Assistant (HCA) to support adults with intellectual and developmental disabilities by coordinating their healthcare needs. In this role, you’ll be a trusted partner and advocate, ensuring timely medical appointments, proper medication management, and seamless communication across care teams. Your support will help individuals lead healthier, more independent lives. Requirements

  • High School Diploma or GED required.
  • Must be at least 18 years of age.
  • Valid driver’s license, insured vehicle, and clean driving record.
  • Ability to speak, read, and hear English fluently.
  • Flexibility to work varied hours as needed.
  • Essential physical skills – be able to run, squat, stoop/bend, kneel, climb stairs, be able to lift 50 pounds, push and pull against potential resistance; and be able to implement non-physical and approved physical interventions and CPR with people supported when needed.

  • Experience supporting individuals with disabilities preferred.

Responsibilities

  • Schedule, transport, and attend medical, dental, vision, and specialist appointments.
  • Maintain annual medical calendars for each person supported.
  • Communicate medical information in a calm, reassuring manner to the individuals you support.
  • Gather information from nurses, house managers, and support staff to ensure effective treatment planning.
  • Accurately document appointment outcomes and provider instructions in the appropriate system.
  • Share updates with interdisciplinary team members, including family members as needed.
  • Enter follow-up appointments and reminders into systems and calendars.
  • Fill medication orders and update medication administration records (MAR).
  • Ensure timely pickup, delivery, and administration of medications.
  • Conduct spot checks on PRN meds, MAR books, and med storage to ensure compliance.
  • Coordinate with pharmacies and obtain prior authorizations when needed.
  • Provide in-home sick care and respond to health-related emergencies, including ER transport.
  • Prepare individuals for medical procedures or tests with appropriate supports.
  • Ensure alternative coverage for appointments if you are unavailable.
