About The Position

The Resident Services Director/ Health and Wellness Navigator is responsible for building relationships with residents and families, solving problems, and being a resource for residents to maximize their highest level of health and well-being. The RSD Health & Wellness Navigator is responsible for evaluating residents’ needs and assisting them in accessing the available resources within and outside the community to reduce barriers for successful independent living. The RSD Health & Wellness Navigator ensures a seamless transition between appropriate levels of care offered at the community by building relationships, solving problems and locating resources for residents transitioning through the continuum of care. The RSD Health & Wellness Navigator will also assist residents in facilitating their wellness and healthcare needs while ensuring social integration into the community. Responsible for coordinating programs and activities designed to help meet the wellness needs of the residents and to develop future programs including intellectual, occupational, social, environmental, spiritual, emotional, nutritional and physical aspects of wellness. Requirements

  • Active Nursing or Social Work licensure for Texas is required.
  • Excellent observation and communication skills, both written and verbal.
  • Demonstrated ability to manage and work with minimal direct supervision.
  • Demonstrated capacity for constructive collaboration and role flexibility.
  • Demonstrated ability to problem-solve, make ethical decisions, and define appropriate boundaries.
  • Demonstrated compliance with accepted professional standards of practice.

Nice To Haves

  • Three years of management experience in a related senior living or healthcare organization preferred.

Responsibilities

  • Provides leadership in the development of objectives, standards of care, and policies and procedures affecting the resident experience particularly in the areas of health and wellness.
  • Leads the weekly Collaborative Care Meeting and works with the multi-disciplinary team to strategize for optimal resident outcomes, campus flow, and occupancy.
  • Participates in leadership meetings, responsibilities, strategic planning and Manager on Duty assignments; actively supports overall community needs.
  • Oversees the Carillon Cares program, including hiring, supervising, and evaluating caregivers to ensure high-quality service delivery, sound business practices, and alignment with organizational standards.
  • Oversees the Receptionist Team for Pointe Plaza and Windsong, including hiring, supervising, and evaluating receptionists to ensure high-quality service delivery and alignment with organizational standards.
  • Leads transition evaluations and aging in place discussions with the residents and family (when appropriate).
  • Ensures residents receive appropriate supportive services or transitions within the continuum of care when their needs change.
  • Partners with the Executive Director to address challenges related to residents at risk.
  • Collaborates with the CH Admissions Coordinator to oversee post-acute discharge planning and follow-up support.
  • Initiates and leads new resident orientation for Independent Living.
  • Performs admissions assessments for prospective residents.
  • Encourages resident participation in community programs and social events.
  • Maintains emergency medical information for each resident, updates annually, and ensures confidentiality of all records.
  • Maintains thorough and confidential documentation appropriate for Independent Living.
  • Reviews emergency call notes and incident reports, implementing strategies tailored to each resident to reduce risks.
  • Oversees registration for Electric Medical Equipment.
  • Assists residents, families, and community teams in preparing for and navigating emergency situations.
  • Acts as a resource for supportive options to improve residents’ health, function, and lifestyle.
  • Assists residents in achieving personal health and wellness goals, incorporating the 8 Dimensions of Wellness and honoring personal choice and mind-body-spirit connection.
  • Initiates resident and/or family meetings (as appropriate) to address issues, reduce risks, and enhance resident well-being.
  • Proactively builds strong, trusting relationships with residents, fostering connection, satisfaction, and positive community culture.
  • Communicate effectively with families, residents, and staff to support resident-centered outcomes.
  • Partners with Life Enrichment Director and Wellness Coordinator to plan, implement and evaluate Wellness Programming and health education opportunities.
  • Develops and maintains a referral network of internal and external service providers.
  • Encourages resident engagement in social, educational, and community programs
