About The Position

Manages the day-to-day clinical services of the community to verify that resident's activities of daily living (ADLs) and healthcare needs are met. Verifies residents are treated with respect and dignity and verifies residents are receiving quality care as their needs change.

Requirements

  • Education as required to obtain state nursing license (LPN/LVN or RN).
  • Minimum of 1-2 years relevant experience.
  • Working knowledge of basic technology and electronic documentation.
  • State nursing license (LPN/LVN or RN).
  • Must have valid driver's license.

Responsibilities

  • Verifies consistent quality resident care and service delivery throughout all shifts; this may require assuming the role and responsibilities of a Caregiver or Med Tech/Aide in order to meet the residents' needs and staffing requirements.
  • Assists in the supervision of community-based licensed, certified, and non-certified clinical associates. Leads care associates and community to gain a reputation for best in market ADL care.
  • Provides training and education to direct care staff on an ongoing basis with classroom in-services and situation-specific training.
  • Assists in the maintenance of resident charts and review of documentation performed by clinical staff.
  • Assists in the management of care and services for each resident, evaluates effectiveness, and maximizes the resident's opportunity to remain in their environment.
  • Participates in pre-admission screening of prospective new residents.
  • Conducts Personal Service Plan review and updates, according to Brookdale and state requirements and involving appropriate parties as needed, to ensure residents' service needs are met.
  • Assists Executive Director with resident retention through participation in new resident welcome events, resident referral programs, Creating a Resident Experience (CARE) program, and collaborative care review process.
  • Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community.
  • Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Brookdale Guidelines.
  • Implements and maintains Brookdale operating standards, systems and processes.
  • Monitors and participates in Quality Improvement and Risk Management programs at Safety Committee Meetings, Collaborative Care Meetings and Family Care Conferences.
  • Assists in the scheduling and follow through of physician visits including, but not limited to, dental appointments, eye appointments, therapy, and hospice both in house and scheduled visits out of the community.
  • Completes documentation as required by Brookdale or state regulatory agencies.
  • Communicates information in a timely manner to the appropriate person or agency regarding changes in residents' health/well-being, personal needs, risk management issues, customer service issues, and family or outside health care provider concerns.
  • Routinely evaluates resident transfer and mobility needs, and verifies clinical staff are aware of any changes.
  • Shares on-call for clinical care concerns.
