About The Position
The Health Risk Assessment Wellness Coach conducts health risk assessment results sessions. Assists team members and/or spouses in identifying their health risks, interpreting results, and facilitating personal wellness plans. Is responsible for the delivery of patient care that meets the high quality of care provided by ThedaCare through collaborative practice with all members of the care team. Contributes to and is accountable for the delivery of compassionate, safe care and demonstrated commitment to continuous growth and professional development.
