The main purpose of this position is to provide assistance to Community Clinical Services nurses during immunization clinics under the direction of the Healthy Communities Program Supervisor. This position will be responsible for preparing for clinics, reviewing immunization records, identifying needed vaccines, providing translation services when needed and tracking and following up with children aged 0–4 years who are delayed on immunizations to ensure they are up to date on vaccines. This position will also provide health promotion and disease prevention information to children and adults seeking immunizations. This position will ensure that program participants receive individualized attention and support in their efforts to take advantage of program and community resources. The employee in this position must have a valid driver’s license, a reliable personal vehicle and automobile insurance. This position will be required to work day, evening and weekend shifts as needed and be able to stand, walk, bend and lift up to 25 lbs.
