About The Position

The Health Promotion Coordinator at Health Plan of San Mateo (HPSM) is responsible for connecting members to health education resources and programs. This role involves engaging with diverse communities, supporting health education initiatives, and conducting outreach to improve health outcomes. The coordinator will manage member inquiries, maintain program records, and coordinate community events, ensuring compliance with health education regulations.

Requirements

  • High School Diploma required; Associate degree in a health-related field is desired; Bachelor's degree preferred.
  • One year of experience in a health care delivery system or managed care plan preferred.
  • Experience in a customer service environment with diverse populations.
  • Knowledge of health promotion and behavior change programs.
  • Ability to provide excellent customer service and work collaboratively in a team.
  • Attention to detail and ability to adapt to changing priorities.
  • Strong organizational skills and ability to maintain accurate records.
  • Effective communication skills with diverse audiences.
  • Proficiency in Microsoft Office Suite applications.

Nice To Haves

  • Bilingual proficiency highly preferred.

Responsibilities

  • Provide excellent customer service via phone, email, and service desk tickets.
  • Review, route, and track incoming requests for the Population Health Department.
  • Maintain knowledge of Health Promotion Programs and eligibility requirements.
  • Conduct member outreach calls to connect members to health resources.
  • Track outreach data and enrollment in Health Promotion programs.
  • Explain member eligibility and deliver training presentations to HPSM staff.
  • Document frequently-asked questions and maintain accurate records of member interviews.
  • Coordinate HPSM's presence at community events and prepare presentations about health programs.
  • Review and update policies and procedures for compliance with regulatory requirements.
  • Maintain spreadsheets and documentation for the Health Promotion unit.
