About The Position

The Health Program Specialist I is responsible for performing routine public health work within various public health programs. This role involves planning and developing public health education initiatives, providing consultative services, and technical assistance to program staff, governmental agencies, and community organizations. The position is grant-funded and does not offer disciplinary appeal rights, emphasizing at-will employment status.

Requirements

• Bachelor's Degree from an accredited college or university in public health, health promotion, social sciences, or a related field.
• Valid Class 'C' Texas Driver's License.
• Experience with case management and public health knowledge.
• Familiarity with Electronic Medical Records (EMR) systems.
• Strong organizational skills.
• Bilingual in English and Spanish.

Nice To Haves

• Experience in provider phone calls.
• Knowledge of current public health programs, practices, and procedures.
• Knowledge of public health laws and regulations.
• Skill in utilizing personal computers and associated software.

Responsibilities

• Implement evidence-based prevention initiatives to improve community health.
• Conduct field outreach to participate in community initiatives.
• Maintain relationships with community and partner organizations regarding health initiatives.
• Interview individuals to gather demographic and medically relevant information.
• Contact healthcare providers to obtain patient information.
• Counsel patients on risk reduction and behavior modification.
• Collect data for planning and evaluating public health education programs.
• Perform data entry and maintain accurate records.
• Provide public health education to individuals and serve as a resource for health issues.
• Assist in preparing materials for information requests and reports.
• Train personnel as needed.
