About The Position

The Health Plan Liaison supports ProCare Advantage’s Medicare Advantage members and partner skilled nursing facilities across Texas. This full‑time, travel‑required role focuses on provider relations, member experience, care coordination support, and facility liaison responsibilities. The Liaison conducts daily onsite visits, builds strong relationships with SNF leadership, supports prospective members and families, provides plan education, and collaborates with integrated care teams to resolve issues quickly and professionally. This position requires strong communication skills, independence, and the ability to represent ProCare Advantage across multiple Texas markets.

Requirements

  • Minimum 2 years of experience in a healthcare delivery setting serving elderly patients; skilled nursing facility (SNF) experience strongly preferred
  • CNA or LVN certification preferred
  • Active Texas health insurance license preferred, or ability to obtain licensure
  • Experience in healthcare sales, provider relations, outreach, or market development preferred
  • Strong interpersonal, communication, and relationship‑management skills
  • Proficient in Microsoft Office; able to maintain accurate digital documentation
  • Reliable transportation, valid driver’s license, clean driving record, and proof of insurance
  • Suitable home workspace for administrative duties
  • Highly organized, self‑motivated, and able to work independently

Responsibilities

  • Serve as the first point of contact for facility staff, providers, members, and families; resolve inquiries within required timeframes
  • Conduct daily onsite visits to assigned SNFs to build relationships, identify needs, and escalate concerns appropriately
  • Support prospective members and families by answering plan questions and coordinating handoffs to licensed Sales Agents
  • Maintain ongoing communication with members and families to ensure satisfaction and address concerns
  • Partner with facility leadership and integrated care teams to support member needs, retention efforts, and plan objectives
  • Organize and facilitate onsite events to promote ProCare Advantage and educate potential members about Medicare Advantage benefits
  • Provide ongoing education to facility staff regarding plan processes, benefits, and care coordination workflows
  • Engage with members considering disenrollment; identify concerns and support retention strategies
  • Build and maintain strategic relationships with key providers and facilities to support network adequacy and growth
  • Administer quarterly member satisfaction surveys, analyze results, and coordinate follow‑up actions
  • Document facility visits, member interactions, and disenrollment activities accurately and in compliance with plan standards
  • Provide updated enrollee rosters and ID cards to facility leadership monthly
  • Attend required in‑service training, continuing education, and professional development activities
  • Perform other duties as assigned
