About The Position

The Health Navigator serves as the primary care manager for individuals and families who need services. This role is crucial in facilitating the initial introduction to the organization's services and maintaining ongoing contact with each assigned participant. The Health Navigator is responsible for developing rapport and relationships that foster trust, ensuring that participants feel comfortable and safe throughout their journey. This position requires a proactive approach, as the Health Navigator will provide face-to-face visits and make outreach calls to participants based on their individual needs. A significant aspect of the role involves performing comprehensive barrier assessments to identify and address various obstacles that participants may face. These barriers can include psychosocial health issues, criminal history, physical health concerns, probation or parole status, mental health challenges, job readiness and competency, substance use, DMV issues, family and support systems, child support orders, custody matters, housing instability, tax issues, and access to food, clothing, hygiene items, civil restitution orders, and healthcare services. The Health Navigator will also assist participants in enrolling in public benefit programs such as Medi-Cal, CalFresh, and CalWorks, as well as other medical and social services. In addition to these responsibilities, the Health Navigator will provide health coaching, including training on the use of glucometers and other health-related tools. They will support medical providers and clinical staff as needed in daily clinic operations, ensuring that all required documentation and reports are completed in a timely manner and in accordance with program contracts. Maintaining a clean and safe site is also a priority for this role. Participation in staff meetings, supervision, agency meetings, and staff training is expected to foster a collaborative and informed work environment.
