About The Position

The nurse health navigator functions as part of a cancer site-specific multidisciplinary team to be an advocate, educator and support for a person with cancer concerns. A coordinator of clinical care from screening throughout the continuum of care, assessing, planning, implementing and evaluating cancer care services. Serves as a liaison between physicians and health care providers throughout Sparrow and the community to promote comprehensive care for patients.

Requirements

  • Chemotherapy / Biotherapy Certification (either current or attained within 90 days of hire)
  • Current State of Michigan license to practice as a Registered Nurse in Michigan
  • Oncology Nursing Certification (OCN) within 3 years of hire. Prior to obtaining certification, must maintain 12 oncology specific CEU’s per year.
  • Minimum two years clinical experience within cancer services
  • Degree from an accredited school of nursing
  • Ongoing continuing education directly related to the field of oncology
  • Participation in and completion of basic oncology nursing course
  • Demonstrated teaching and program development ability.
  • Demonstrated interpersonal and communication skills, written and verbal.
  • Demonstrates positive team attitude and has interpersonal skills to effectively and appropriately communicate with clients, families, physicians, and other co-workers

Nice To Haves

  • Oncology Nursing Society (ONS) Certification Preferred.
  • Masters Degree in Nursing Preferred.

Responsibilities

  • Coordinate and provide education, information, and support to cancer patients from screening to survivorship, to help make their care seamless, comprehensive and continuous.
  • Under the guidance and supervision of the Radiology, Pathology, and Oncology physicians, Navigator will share diagnostic results, as appropriate, with the patient
  • Always works in collaboration with the multidisciplinary team at Sparrow and other patient care providers to provide continuous, consistent and comprehensive care
  • Establish and maintain clinical and program databases and provide reports related to patient navigation.
  • Coordinate and evaluate the MDC clinics in collaboration with the multidisciplinary team and medical director
  • Serve as a consultant to patients and families for information, support and referral to community resources
  • Facilitate and plan educational cancer outreach programs for community outreach events and cancer support groups
  • Coordinate ancillary programs for cancer survivors
  • Assist grant writers, as appropriate, in an effort to attain program funding.
  • Participate in appropriate hospital and community committees and activities
