Health Navigator I helps to drive increased engagement and member adherence for key quality program measures. This includes managing multi-modal forms of communication, including phone, sms, email, and portal. This role collaborates with other teams within the organization to deliver care to members in a coordinated and streamlined manner. This role may also interface with providers, health plans, and other partners on an as-needed basis.
