Requirements

  • Minimum of 3–5 years of HIM experience with advanced medical records responsibilities.
  • Demonstrated expertise in EPIC EHR and electronic documentation workflows.
  • Comprehensive understanding of HIPAA, PHI protection, and release of information regulations.
  • Associate degree in Health Information Management or related field (Bachelor’s degree preferred).
  • RHIT or RHIA certification strongly preferred.
  • Notary commission required.
  • Strong oral and written communication skills with the ability to interact professionally with providers, staff, and external stakeholders.
  • Advanced knowledge of medical terminology, HIM principles, and regulatory requirements.
  • Exceptional organizational skills with the ability to manage multiple priorities and meet deadlines.
  • Demonstrated ability to work independently with strong critical thinking, decision making, and problem solving abilities.
  • Expert proficiency in EPIC EHR, including chart corrections, work queue management, and documentation workflows.
  • High proficiency in Microsoft Office Suite, SharePoint, and Outlook.
  • Strong attention to detail with excellent follow-through and accuracy.

Responsibilities

  • Advanced Documentation Management Monitor, track, and resolve incomplete, unsigned, and delinquent documentation, including operative reports, highrisk encounters, and timesensitive clinical documentation. Collaborate with providers to ensure timely completion of required documentation and escalate issues as appropriate.
  • Chart Integrity & Auditing Conduct advanced chart analysis, including chart merges, demographic corrections, and documentation audits to ensure accuracy and compliance. Investigate and resolve chart discrepancies, duplicate records, and data integrity issues, document all actions in accordance with organizational and regulatory standards. Support internal and external audits by preparing documentation, validating data, and ensuring audit readiness.
  • Release of Information (ROI) Process complex and sensitive ROI requests, including legal, insurance, and billing-related disclosures, ensuring strict adherence to HIPAA and organizational policies. Manage escalated or highrisk requests, including subpoenas, attorney inquiries, and specialized disclosures. Notarize medical records as required and authorized.
  • EHR Expertise Utilize advanced EPIC EHR tools for work queue management, chart corrections, imaging normalization, and documentation workflow optimization. Serve as a subject matter expert for EPIC documentation processes and HIM-related system functionality.
  • Compliance Leadership Ensure full compliance with federal and state regulations governing PHI, including HIPAA, HITECH, and organizational privacy policies. Act as a resource for staff and providers regarding privacy, security, and documentation best practices.
  • Imaging & Data Integration Oversee normalization and integration of incoming imaging, external medical records, and thirdparty documentation. Coordinate with external facilities and vendors to ensure timely and accurate data exchange.
  • Provider & Patient Support Assist providers with medical record requests, documentation troubleshooting, delinquent records, and unsigned documentation. Provide exceptional customer service to patients, providers, and internal departments regarding medical record inquiries and documentation needs.
  • CrossFunctional Collaboration Partner with revenue cycle, clinical operations, IT, and external vendors to support audits, billing documentation requirements, and workflow improvements. Participate in interdisciplinary initiatives to enhance documentation quality and operational efficiency.
  • Process Improvement Identify gaps in HIM workflows and recommend process enhancements to improve accuracy, efficiency, and compliance. Contribute to policy development, training initiatives, and continuous quality improvement efforts.
