About The Position

The HIM Specialist is responsible for maintaining accurate, complete, and timely medical records in both electronic and paper formats. This role ensures patient documentation is properly compiled, verified, and processed to support quality care, regulatory compliance, and operational efficiency. Responsibilities and expectations may vary based on experience and demonstrated performance, with more experienced specialists handling complex records, assisting with audits, analyzing trends, and providing guidance to other team members.

Requirements

  • High school diploma or equivalent preferred.
  • AHIMA credentials (RHIT, RHIA, CCS, CCS-P) or AAPC credentials (CPC, CPC-H) required or eligible for credential testing.
  • Commitment to ongoing education in healthcare coding.
  • Specialist I: Entry-level to foundational HIM experience; supports routine record management tasks.
  • Specialist II: Demonstrated experience handling complex record management issues; supports audits and compliance initiatives.
  • Specialist III: Typically 4–7 years of hospital acute care or multi-specialty clinic experience; serves as a subject matter expert and contributes to process improvement and staff guidance.
  • Equivalent skills and demonstrated performance may be considered in lieu of years of experience.
  • Experience with electronic health record (EHR) systems required; experience with specific software platforms preferred.

Responsibilities

  • Maintain medical records by reviewing, verifying, and organizing patient information in accordance with hospital policies and procedures.
  • Accurately scan and index documents into the electronic health record (EHR) system.
  • Maintain and update the master patient index (MPI) to ensure correct patient identification.
  • Assign chart deficiencies and resolve discrepancies by collecting and analyzing data from patient records.
  • Input patient demographics, diagnoses, and treatment details with a high level of accuracy.
  • Process requests for protected health information (PHI) from patients, departments, healthcare providers, and legal entities while adhering to privacy regulations.
  • Follow release-of-information policies and legal requirements to protect sensitive health information.
  • Communicate professionally with providers, peers, and third parties regarding medical records and documentation.
  • Collaborate with other departments to collect missing documentation and ensure complete patient records.
  • Prepare statistical reports summarizing diagnoses, treatments, surgeries, and hospital utilization.
  • Adhere to HIPAA regulations and maintain patient privacy at all times.
  • Provide excellent customer service and foster strong working relationships within the HIM department and across the organization.
  • Perform a variety of tasks and other duties as assigned.
  • Handle more complex record management tasks, including auditing, discrepancy resolution, and problem-solving.
  • Support quality improvement initiatives and help ensure regulatory compliance in all records.
  • Analyze trends in documentation and reporting to identify opportunities for process improvements.
  • Assist with training, mentoring, and guidance for less experienced HIM staff.
  • Collaborate with internal teams to improve workflows, record accessibility, and overall HIM efficiency.
