Job Summary: The Health Information Coordinator plays a critical role in ensuring the security and compliance of patient records. This position directly impacts the organization’s ability to adhere to privacy regulations, maintain and protect patient confidentiality.

Requirements

  • High school diploma is required, associate or bachelor’s degree or Certification in Health Information Management or related field preferred.
  • Minimum of one year experience in Health Information Management or Medical Record Production, or a proven track record of strong performance evaluation rating required.
  • Expertise of HIPAA, Part 2, and state privacy laws regulations required.
  • Proficiency in using computer applications, including navigating Electronic Health Records (EHR) systems.
  • Strong verbal communication skills, with a proven ability to respond to patients, clinicians, clients, and vendors in a timely manner.
  • Demonstrated diligence in adhering to privacy rules.
  • Strong organizational skills and the ability to manage large volumes of record requests and maintain accurate tracking and documentation.

Nice To Haves

  • Certification(s) a plus: Certification in Health Information Management (CHIM) or Certified Information Privacy Professional (CIPP), Health Information Management Certificate, Release of Information (ROI) certification, Certified in Healthcare Privacy Compliance (CHPC), Certified Professional in Health Information Management Systems (CPHIMS), Registered Health Information Administrator (RHIA), Certified HIPAA Professional (CHP).

Responsibilities

  • Organize and maintain patient record requests, including internal request management, tracking and coordination.
  • Ensure all requests are processed in compliance with applicable laws and regulations.
  • Manage medical record requests with internal privacy notations which includes minor records, confidential/my eyes only, litigation, substance use disorder records.
  • Collaborate with internal staff to verify no safety concerns and ensure proper consent from all parties around medical records releases.
  • Review records request to determine which records to be released in accordance with HIPAA, Part 2, and state privacy guidelines.
  • Obtain relevant pertinent patient data/records from legacy systems/sources, including electronic, off-site, or physical records.
  • Communication with stakeholders via phone and email concerning various ROI issues.
  • Process requests utilizing enterprise medical record systems and platforms.
  • File and retrieve patient charts and medical documents in a timely manner, ensuring easy access for healthcare providers and staff.
  • Strictly adhere to privacy laws, including HIPAA, Part 2, and state privacy laws, confidentiality, and privacy of patient information.
  • Review medical records for completeness, accuracy, and compliance with organizational standards and policies.
  • Coordinate and support the Company's outside medical records vendor as necessary, ensuring timely and accurate processing of medical records requests, compliance with regulatory requirements, and adherence to company policies and procedures.
  • Assist with business and facility licensure and maintenance.
  • Perform other duties as assigned in alignment with LifeStance’s values.
