About The Position

Mountain View Hospital is seeking a Health Information Management Clerk to join their team. This role involves reviewing medical records post-discharge to ensure completeness in compliance with hospital bylaws, federal, and state requirements. The clerk is responsible for accurately entering patient abstracts into the chart deficiency system, monitoring daily admissions for duplicate medical record numbers, and verifying patient discharge status during chart assembly. Key duties also include timely filing of transcribed reports, communicating with physicians regarding delinquent reports, and ensuring the accurate transcription of dictated reports. The position requires familiarity with state guidelines for processing charts, including timeframes for History and Physical and Operative reports, as well as knowledge of Medical Record content, maintenance, and retention. Requirements

  • One to three months related experience and/or training; or equivalent combination of education and experience.

Nice To Haves

  • High school diploma or general education degree (GED).

Responsibilities

  • Reviews medical records after discharge for completeness in compliance with the hospitals bylaws, federal and state requirements.
  • Assures that patient’s abstracts are correctly entered into the chart deficiency system.
  • Monitors accounts being admitted daily for duplication of medical records numbers, along with checking patients discharge status when assembling the chart.
  • Ensures that all transcribed reports are filed in a timely manner in the charts.
  • Communicates with physicians about delinquent reports.
  • Accountable for the accurate transcription of dictated reports.
  • Assures the completion of incomplete medical records within the time frame as specified by the medical staff rules and regulations.
  • Is familiar with requirements of the state guidelines of timeframes for processing of the charts such as the History and Physical and Operatives report timeframes allowed.
  • Knows Medical Record content, maintenance and retention of records
