About The Position

As a Health Information Coordinator at Frasier, a nonprofit 501(c)(3) Life Plan retirement community rooted in Boulder since 1960, you will be a crucial partner for clinical teams, ensuring medical records are accurate, organized, and compliant. This role is vital for both regulatory compliance and the quality of care provided to residents across Frasier’s continuum of care, which includes independent living, assisted living, memory support, and skilled nursing. The position involves supporting accurate documentation, managing health information processes, and being a reliable resource for clinical teams and leadership. Requirements

  • Experience in health information management, medical records, or related healthcare roles
  • Working knowledge of electronic health record systems
  • Familiarity with HIPAA privacy standards and medical record documentation requirements
  • Familiar with ICD-10 coding and its application to medical record documentation

Nice To Haves

  • Experience in skilled nursing or long-term care preferred
  • Working knowledge of PointClickCare preferred
  • Associate degree in Health Information Management, Health Administration, or a related field preferred
  • RHIT, RHIA, CHIM, or similar credential preferred but not required

Responsibilities

  • Maintain day-to-day health information processes across Skilled Nursing, Assisted Living, and Memory Support
  • Ensure documentation is complete, timely, secure, and survey-ready
  • Monitor records in PointClickCare (PCC) and follow up on missing or incomplete documentation
  • Conduct routine medical record audits and identify opportunities for improvement
  • Partner with clinical leadership to support documentation standards and regulatory readiness
  • Assist with Quality Assurance and Performance Improvement (QAPI) efforts
  • Manage release of health information in compliance with HIPAA and organizational policy
  • Maintain disclosure logs and oversee record retention and secure destruction
  • Serve as a knowledgeable and approachable resource to clinical teams, admissions, and leadership
  • Support survey readiness and coordinate record retrieval during audits or reviews
  • Collaborate with an RHIA consultant providing oversight of the Skilled Nursing medical record program
